The Clinical Advocate coordinates care for defined sleep medicine and balance disorder patient population to include but not limited to: Ongoing assessment of patients’ needs - educational, psychosocial, nutritional and clinical. Connects patients and families to available resources within the health system & community. Assesses patients for eligibility for services and facilitates access to care. Screens both inpatients and outpatients to identify risks for sleep related disorders, balance and vestibular deficits. Offers navigation services by reaching out to physicians and connecting the patients with the appropriate diagnostic treatment services.
